What kind of application is this?

Add a person to an existing active affiliate company Our company is already a current WMBR affiliate. We're adding another individual. Reactivate an inactive prior affiliate company Our company was an affiliate before but isn't currently active. We want to rejoin (with one or more individuals). Brand new affiliate company Our company has not been a WMBR affiliate before. We're joining for the first time (with one or more individuals).
